Athletics at Robert Eagle Staff

Winter Sports – Basketball starts week of December 5

Girls Tryouts are December 7th and 8th from 4:00 to 5:45

Practices will start 12/12 on Mondays and Thursdays

Boys Tryouts are December 11th and 12th from 4:00-5:45

Practices will start 12/13 on Tuesdays and Fridays – pending update on early release day

Register through FinalForms.

Registration must be complete, including a valid sports physical signed by a doctor, prior to any participation. Sports physicals can be uploaded or turned in to the RES main office.

There will be no practices when school is closed for breaks, holidays, due to weather or if otherwise notified

Robert Eagle Staff MS participates in all Middle School interscholastic sports.

  • Fall: Girls Soccer, Ultimate Frisbee
  • Winter: Girls & Boys Basketball
  • Spring: Girls Volleyball, Boys Soccer, Co-ed Track

All Athletic registration is done online using FinalForms.

  1. Go to FinalForms and create your account (you will add your student later)
  2. Locate the parent icon and click New Account below.
  3. Type your Your Name, Date of Birth, and Email, and Email. Next, click Register.
    Note: You will receive an email within 2 minutes prompting you to confirm and complete your registration. If you do not receive an email, then check your spam folder. If you still can not locate the FinalForms email, then email support@finalforms.com informing our team of the issue.
  4. Check your email for an Account Confirmation Email from the FinalForms Mailman.
    Once received and opened, click Confirm Your Account in the email text.
  5. Create your new FinalForms password. Next, click Confirm Account. Once account is confirmed, you can register your students.
  6. Click Register Student for your first child. You will need basic medical history and health information. Insurance company and policy number. Doctor, dentist, and medical specialist contact information. Hospital preference and contact information.
  7. Locate and click the Add Student button.
  8. Type in the Legal Name and other required information. Then, click Create Student.
  9. If your student plans to participate in a sport, activity, or club, then click the checkbox for
    each. Then, click Update after making your selection. Selections may be changed until the
    registration deadline.
  10. Complete each form and sign your full name (i.e. ‘Jonathan Smith’) in the parent signature field
    on each page. After signing each, click Submit Form and move on to the next form.
  11. When all forms are complete, you will see a ‘Forms Finished’ message.
    Important: If required by your district, an email will automatically be sent to the email address that you provided for your student that will prompt your student to sign required forms.
  12. You can repeat this process for your other students. Already have a student registered for High School athletics in Seattle? You just need to add your middle school student. Login at any time during the year and click Update Forms to update information for any student

All athletes must have a current sport’s physical signed by a doctor, along with all other required forms, prior to any participation. The sport’s physical form is located on the SPS District website

Athletic Registration

We are using FinalForms for online athletic registration. If you previously had an account, you can log in and all your info will still be there!

If you have not created an account, please follow the instructions below. A parent/guardian must create their FinalForms account first, before the student is able to create their account.

  1. Parents/guardians, go to the FinalForms website
  2. Type your name, date of birth, and email and then click register
  3. Check your email for a FinalForms email, and click confirm your account in the email text.
    • You will receive an email within 2 minutes prompting you to confirm and complete your registration. Remember to check your spam folder. If you still do not see the FinalForms email, please email support@finalforms.com.
  4. Create your new FinalForms password and click confirm account.
  5. Click register student for your first child. Add your student and their email.

If you require any support during the registration process, scroll to the bottom of the FinalForms page and click “Use Support”.

Physicals are valid for 24 consecutive months

  • The History document (Page 1) should be completed and remain with your provider
  • The Exam document (Page 2) should be completed and remain with your provider
  • The Medical Eligibility document (Page 3) should be the only item submitted to your school

Student-athletes must have health insurance to play a sport. If not covered by a parent/guardian’s insurance plan, insurance can be purchased through SPS.

Digital Tickets for High School Games

SPS home games (matches played at SPS high schools) can be purchased online via SPS HomeTown Ticketing Box Office.

This includes all regular season games for football and basketball. It also includes post-season games for volleyball, basketball, wrestling, gymnastics, baseball, soccer, softball (slowpitch and fastpitch), and track/field. Learn more about SPS Digital Athletic tickets.

Please note: tickets are posted under the HOST school (For example, West Seattle High School hosting Garfield High School would appear under West Seattle.)
