Robert Eagle Staff School-Based Health Center
The Robert Eagle Staff School-Based Health Center (SBHC) is a full service, in-school health clinic that provides physical and mental health care to the students of Robert Eagle Staff Middle School. The SBHC is operated by NeighborCare Health and partially supported by the Families and Education Levy and the City of Seattle.
The SBHC offers a student friendly setting and all the services (and more) of a family doctor, including immunizations, sports physicals, and health education. Our Mental Health services are provided on an individual and group basis and include treatment for depression, anxiety, stress and much more.
We accept medical coupons and most types of insurance. No student is denied service due to inability to pay. An eligibility specialist is also available who can help patients sign up for insurance. To access all the offered services, complete the registration form and return it to the Robert Eagle Staff SBHC.
Open during regular school days for appointments. When the school is closed, for example during holiday breaks and over the summer, students are encouraged to contact our Meridian Clinic at 206-296-4990.
Register
Any student enrolled at Robert Eagle Staff Middle School can be seen by our providers by having a parent or guardian complete and sign the registration forms and return them to the health center. They can either be delivered by the student or parent or placed in a secure envelope and mailed to the health center’s address (listed above). Printed copies of the registration forms are also available in the health center. Under state law, youth may independently access some health care without parental consent, including reproductive health and mental health services.
We now have online registration through DocuSign. Simply follow the prompts and you will be able to virtually sign and fill out the registration forms.

Before or after school appointments are available. We will attempt to schedule clinic appointments so students miss as little class time as possible.
Cost
No student will be denied services due to inability to pay. Because the Families and Education Levy covers only a portion of the cost to operate the Robert Eagle Staff SBHC, we will bill insurance companies when possible.
For students on Medicaid or a public insurance plan, we bill all services directly and the student / family will have no out-of-pocket expense. For students with private insurance, it is possible that parents / guardians may have some out-of-pocket cost, depending on the type of services and on the private insurance company’s coverage. Not all services are subject to insurance company policies, so please call if you would like more information.
Neighborcare Health can assist uninsured or under-insured students and their families with completing insurance enrollment paperwork. All you need to do is indicate that you have no insurance under the insurance section of the registration form, and we will follow up with you.
